How PF and ESI apply to your staff
PF (Provident Fund, also called EPF) is savings and a pension for employees. ESI (Employees' State Insurance) gives them medical care. PF applies once you have 20 or more employees. ESI applies once you have 10 or more.

Nurses and support staff
Many nurses, ward staff and helpers earn ₹21,000 a month or less in ESI wages, so they must be in ESI.
ESI wages are the month's pay minus some items, like HRA (house rent allowance) and overtime. But ESI wages must be at least half the total pay. If they come to less, the difference is added to ESI wages. Shift and night-duty allowances usually count. So ESI wages can change each month.

Agency staff
Housekeeping, security and some nursing staff often come from an agency. If the agency doesn't pay their PF and ESI, the hospital can be held responsible. So ask the agency for proof every month: their challan (the PF or ESI payment slip).
Doctors
Visiting and consultant doctors
Some doctors work as consultants, not staff. But if a doctor works like staff, EPFO (the PF office) may count them as staff. Then it can ask for PF. Call us to check how to count your doctors.
When staff don't want to join
Nurses and ward staff sometimes ask to skip PF or ESI to take home more pay. If the law covers them, they must join. They can't say no.
It helps to tell them what they get. ESI gives them and their family free medical care, and cash help when they are sick or on maternity leave. PF builds savings, a pension and life insurance. We can help you explain this to your staff.

What we do for you
You send us salary details. We do the PF and ESI work.
- We register your hospital for PF and ESI, if it isn't registered yet.
- We add new staff to PF and ESI, if the rules cover them. Staff who already have a UAN (a PF account number for life) keep it. Others get one by a face scan on the UMANG app (the government's mobile app). We guide them. Staff who already have an IP number (their ESI number) keep it. We create one for the others.
- We work out ESI wages each month, as shift pay changes.
- Each month, we file your PF and ESI returns (the monthly list of each employee's wages and the PF or ESI on them, sent to the PF and ESI offices), and mark the date when staff leave. We email you the challans (the payment slips). Pay them by the 15th of the next month, or send us the amount and we pay it.

Common questions
Yes. If an agency or contractor sends workers to you, they count. They count towards the 10 (ESI) and 20 (PF) limits. Usually the agency, as their employer, pays their PF and ESI. Ask the agency for their challans every month.
It depends on how they work with you. If a doctor works like a staff member, they may count as an employee. Then they count towards your 10 and 20, and may have to be in PF. Call us and we'll check each case with you.
Not straight away. ESI works in two six-month periods: April to September, and October to March. If a nurse's ESI wages go above ₹21,000 during a period, they stay in ESI until it ends. You keep paying ESI for them until then.
ESI applies once a clinic has 10 or more employees, and PF at 20 or more. Count all staff, including casual workers and agency staff. Have part-time staff? Call us to check how to count them.
